This is a modified version of the Black Tie theme by Heather Rivers. The main difference is that you can now change the theme’s colours with Tumblr’s colour widget in the customization menu. This took some hacking of the CSS so I can’t guarantee it’ll look pretty in every possible browser but it was tested in Firefox 3, Opera 9 and Internet Explorer 7 and looked fine in all of them.
This theme is shared under a Creative Commons licence. You can do whatever you want with it as long as you share derivative work, don’t make money off it and give credit where credit’s due.

UPDATE: If the title font is too large for your title, you can put the following line in the Custom CSS box in the advanced customization tab:
div#header h1 {font-size: 60px;}
Replace the 60 with a smaller number for an even smaller font size.
